Ultimately, medicines optimization can help to encourage patients to take ownership of their treatment. However, the medicines optimization approach requires multidisciplinary team working to an extent that has not been seen previously. Healthcare professionals need to work together to individualize care, monitor outcomes more carefully, review medicines more frequently and support patients when needed (NICE [247], RPS [314]). Figure 15.1 shows how this works.
