Regular, accurate and in‐depth wound assessment documentation is vital in monitoring wound healing and evaluating the effectiveness of a treatment plan (Baranoski and Ayello [8]). Regular wound measurements and photography can be helpful in communicating wound status to the multidisciplinary team and reducing unnecessary dressing changes.
Frequency of dressing changes should be carefully considered and based on the wound type, exudate level and dressing properties (NICE [80]). High dressing change frequency should prompt a treatment plan reconsideration.
